Call Sapphire before requesting records to confirm current patient acceptance and insurance information. Sapphire’s current process requires previous medical records before a new-patient or well-check appointment can be scheduled for an incoming child who is not a newborn.

Request a complete record

Ask the previous practice to send growth charts, immunization history, medication and allergy lists, relevant birth or hospital records, laboratory and imaging reports, specialist notes, and developmental, behavioral, school, or therapy information held in the medical record. Complete the previous practice’s release process and allow processing time.

Prepare a current summary

Bring a one-page summary of diagnoses, medications and doses, allergies and reactions, specialists, surgeries, hospitalizations, preferred pharmacy, and your goals for transferring. If a medication cannot be interrupted, begin the transfer before it runs low. A new clinician may need records and an evaluation before prescribing.
