Download the form you need, complete all parent or guardian sections, and follow Sapphire’s instructions for secure return. A form is not complete until all required sections have been reviewed and signed by the appropriate person.
Some forms require a current examination, medication review, additional records, or processing time. A clinician cannot sign a statement that is not supported by the medical record. Do not send completed forms containing health, insurance, payment, or other sensitive information to the general administrative email unless Sapphire specifically directs you to an approved secure method.
Auto Pay Authorization
Auto Pay authorization is completed electronically at check-in. The front desk will swipe the card; card information is no longer collected on a paper form.
