Ask whether Sapphire Pediatrics is in network for your exact plan and what the plan requires for pediatric primary care, sports medicine, behavioral health, procedures, vaccines, referrals, laboratory services, or imaging.
Know what your plan decides
Your insurance company determines benefits and processes claims. A network statement or estimate is not a guarantee of payment. Families remain responsible for understanding deductibles, copayments, coinsurance, noncovered services, prior authorization, referral rules, and balances under the practice’s current financial policy.
Prepare for the visit
Bring current insurance information and photo identification to the visit, and notify the office promptly when coverage changes.
