Nondiscrimination
Sapphire Pediatrics provides services without unlawful discrimination. We do not exclude people or treat them differently because of race, color, national origin, age, disability, sex, religion, ancestry, marital status, sexual orientation, gender identity or expression, or another status protected by applicable law.
When required by law, Sapphire provides reasonable modifications and appropriate auxiliary aids and services to people with disabilities to support effective communication. These services may include qualified interpreters or information in alternative formats.
When required by law, Sapphire provides language-assistance services free of charge to people whose primary language is not English. These services may include a qualified interpreter or translated information. Family members, friends, and especially children should not be required to interpret except in limited circumstances allowed by law.
To request an interpreter, auxiliary aid, alternative format, or reasonable modification, call 720-941-1778. If possible, contact the office before the appointment so arrangements can be made. A request will not reduce the quality of care or result in retaliation.

Describe the concern, the date, the people involved, and the resolution you are requesting. Do not include sensitive medical information in ordinary email. Call the office to ask for a secure way to provide supporting information.
You may also file a civil-rights complaint with the U.S. Department of Health and Human Services Office for Civil Rights through its online complaint portal, by mail to U.S. Department of Health and Human Services, 200 Independence Avenue, S.W., Room 509F, HHH Building, Washington, D.C. 20201, or through current contact channels at hhs.gov/civil-rights/filing-a-complaint.
Sapphire will not retaliate against anyone for requesting assistance, raising a concern, or filing a complaint.
Availability of language assistance and auxiliary aids
Free language-assistance services and auxiliary aids and services are available when required. Call 720-941-1778. Sapphire will provide translated notices and communication support as required by applicable law and the needs of the people it serves.
