New patient information

We are accepting newborn patients. Please look at what insurances we do not accept on our website before calling to schedule. If we do accept the insurance you have, please give us a call to schedule your newborn appointment. The newborn appointment should take place a day or two after discharge from the hospital. Please bring your hospital discharge paperwork, insurance card and photo ID to the first newborn appointment. 

We are accepting newborn patients. We do offer meet and greet appointments if you would like to come see our facility and meet with a provider. We also offer virtual meet and greet appointments if you have any questions for one of our providers. If baby is being born at Rose Medical Center, please let your nurse know that you are planning to come to Sapphire Pediatrics, and they will give us a call with all your information. A provider will then come and see you in the hospital all the way up until you are discharged. Once discharged from the hospital, your newborn appointment will be a day or two after. If you are having a baby at any other hospital, please give us a call when you know when you will be discharged from the hospital, and we will get a newborn appointment scheduled a day or two after discharge. Please bring your hospital discharge paperwork, insurance card and photo ID to the first newborn appointment.

Previous medical records are required to schedule a new patient or well check appointment. Please reach out to your previous pediatrician to transfer over previous medical records. Once the medical records are received, we will give you a call to schedule your first appointment.
