Care & Services / School, Sports & Camp Physicals
A thoughtful preparticipation evaluation reviews overall health, identifies concerns that may affect safe activity, and completes required documentation when medically appropriate.

Care built around your child
Schools, sports programs, and camps may require health forms before a child participates. Whenever possible, complete the sports preparticipation evaluation as part of routine health supervision rather than treating it as paperwork alone.
The visit may review personal and family history, medications, allergies, previous injuries, exercise symptoms, nutrition, emotional health, and other concerns. Additional evaluation may be needed before a clinician can sign a form or clear participation.

How this care can help
Complete the parent section and bring the school, team, or camp document.
Include medications, allergies, previous injuries, symptoms, and family history.
New concerns or missing records may require follow-up before signing.
A current injury may need a separate focused sports medicine appointment.
A clear next step
Bring glasses or contacts, braces, inhalers, an updated medication list, prior injury or concussion information, and relevant specialist recommendations. Clearance reflects the information available and the child’s current examination.
