Schools, sports programs, and camps may require health forms before a child participates. Whenever possible, complete the sports preparticipation evaluation as part of routine health supervision rather than treating it as paperwork alone.

The visit may review personal and family history, medications, allergies, previous injuries, exercise symptoms, nutrition, emotional health, and other concerns. Additional evaluation may be needed before a clinician can sign a form or clear participation.

Not always. The clinician must have enough current information to make a safe decision. Missing records or a new concern may require follow-up.

Is a sports physical the same as a well visit?

No. There is overlap, but a complete well visit includes broader preventive care and age-appropriate screening.

How long is a physical valid?

Requirements vary by organization. Check the form and deadline with the school, league, or camp.
